Information from Jasper Unit 1 on the first week of school and COVID check in processes.
Parents:
As you may be aware, unfortunately, the COVID Check-In app being built for our district is not ready quite yet. It is currently with Google and Apple for approval before it can be added to the app stores. (Being categorized a health-related app, it is subject to more scrutiny and review than a normal app.) As a result, we will be using the form linked below to do symptom screenings and temperature recording in the meantime.

Please fill this form out once each day for each student attending school. It is NOT necessary to input screenings for remote learners into this form.
If your child(ren) are riding a bus, we will also need the attached form printed out each day (in addition to the online form linked above), completed, and given to the bus driver by your student when the get on the bus to certify that students are symptom-free. If you do not have a printer, please contact the Transportation Office at 618-783-3023 to make arrangements to pick up printed copies of the form.
